Loss of motive power due to engine failure post recall remedy

The Office of Defects Investigation (ODI) has received 36 Vehicle Owner Questionnaires (VOQs) alleging engine failure in vehicles subject to Recall 25V274. In each VOQ, the engine had the recall remedy completed prior to the reported failure incident. Recall 25V-274 addresses vehicles equipped with a L87 6.2L V8 gas engine that experienced engine damage or failure. Recall 25V-274’s remedy includes an engine inspection.  Based on the results of the inspection the vehicle will receive an oil change to a higher viscosity oil or an engine replacement.  The complaints received allege failures of both remedies. ODI is opening this Recall Query (RQ) to assess the adequacy of the remedy for Recall 25V-274. The Office of Defects Investigation (ODI) has received 1,157 reports of engine bearing failure failures in the L87 6.2L engine produced by General Motors (GM). Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019–2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. The subject engine failures were investigated as part of a Preliminary Evaluation (PE25001).  GM determined that these engine failures were due to multiple supplier manufacturing and quality issues. GM issued recall 25V-274 to address the supplier manufacturing and quality issues produced between March 1, 2021, and May 31, 2024. Based on the issuance of recall 25V-274, NHTSA has closed PE25001. ODI continues to receive a significant number of reports of engine failure in vehicles outside the scope of 25V-274.  This presents a potential safety risk that warrants further investigation.  ODI is opening this Engineering Analysis (EA) to further evaluate the scope and severity of the potential problem and to fully assess the potential s

On January 16, 2025, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE25001 after receiving complaints and field reports concerning engine failures in MY2019-2024 GM’s full-size truck and SUV T1XX platform equipped with the L87 6.2L V8 engine. These complainants alleged engine bearing failures resulting in knocking and growling noises from the engine’s bottom end and in some cases more severe consequences such as broken connecting rods leading to engine damage, or engine failure. If the engine fails during vehicle operation, the vehicle will lose propulsion, increasing the risk of a crash. On April 24, 2025, GM issued recall 25V-274 to address the concern with L87 engines produced between March 1, 2021, and May 31, 2024.  GM cited the root cause as a supplier manufacturing and quality issues. GM’s  recall remedy will be either an oil change to an oil with a different viscosity or engine replacement depending on the results of the remedy inspection procedure. Electronic Brake Module Component Failure

The Office of Defects Investigation (ODI) has received multiple Early Warning Reporting field reports alleging a loss of brake boost assist in MY2023 Cadillac LYRIQ vehicles. The reports allege a hard brake pedal, followed by a “Brake System Failure” message at start up or while driving. The EWR field reports have been verified by General Motors (GM). GM stated the electronic brake control module, also known as “eBoost”, has an internal spindle that can fracture during an Anti-Lock Braking System (ABS) event. GM further stated that due to the fracture, the vehicles can lose brake assist, experience speed limitation, and ABS would lose some functionality. GM indicated a new material clean break point was introduced in the assembly plants in May 2023.  GM has also developed an over the air (OTA) update, which will cycle the brake booster similarly to an ABS event at start-up for every key cycle. If the spindle fractures during this start-up cycling, a warning lamp will illuminate to prompt the customer to bring the vehicle to the dealership to be diagnosed and repaired under warranty. Rear-View Camera Failure

On July 22, 2022, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE22-006) to assess alleged failures of the rear-view camera (RVC) in Model Year (MY) 2020-2021 Cadillac XT5, XT6, and GMC Acadia vehicles. The complaints report that the screen for the RVC is black, sometimes with a red triangle and a circle with a line through it. To date, ODI has received a total of 18 complaints and several EWR Field Reports with allegations of the RVC image failing to display when the vehicle is in reverse. The reports have been received over the past 28 months with most occurring with low vehicle mileage.During the investigation, ODI learned incorrect crimping specifications were utilized in the production of the coaxial cable connector. Improper crimping leads to loss of connection between the RVC and dashboard screen that results in failure of the RVC image to display. Failure or malfunction of the RVC image to display results in reduced visibility of the area behind the vehicle. This limited view may lead to an increased risk of incidents resulting in injury and/or property damage while the vehicle is in reverse.The improperly crimped connectors were only present o

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. Air Bag Malfunction

On April 9, 2021, the Office of Defects Investigations (ODI) opened PE21-010 to investigate instances of air bag malfunction indicator light (MIL) illumination and potential for frontal air bag non-deployment in several vehicles manufactured by General Motors (GM). The vehicles included model year (MY) 2020 through 2021 CT4 and CT5 passenger cars, Sierra and Silverado trucks (1500/2500/3500 models), XT4 and MY 2021 Escalade, Suburban, Tahoe, Yukon SUVs.Technical Service Bulletin (TSB) 21-NA-005, issued in March 2021, addresses air bag MIL illumination with diagnostic trouble codes (DTCs) B0001-1B or B0012-0D. It cites rust particles in the connection terminal interface of the driver air bag inflator housing as the cause of the air bag MIL illumination. Illumination of the air bag MIL under these circumstances may result in a non-deployment of the frontal driver air bag during a frontal collision and increased risk of injury to the driver.GM stated a correlation exists between production delays due to the pandemic shutdown and premature corrosion initiation on the surface of the air bag inflator housing, which, in some cases, caused rust development during a seven (7) month period o

Brake Vacuum Pump Failure

On November 20, 2018, the Office of Defects Investigations (ODI) opened PE18-012 to investigate instances of sudden onset of a hard brake pedal that could result in extended stopping distance due to an issue with the brake assist vacuum pump in model year (MY) 2014 through 2016 GMC Sierra 1500 and Chevrolet Silverado 1500 series pickup trucks and MY 2015 through 2016 Cadillac Escalade, Chevrolet Suburban, Chevrolet Tahoe, and GMC Yukon SUVs.If the brake assist vacuum pump fails to operate as intended, the amount of brake power assist supplied to the driver can be significantly reduced, extending the subject vehicle stopping distance. GM stated that the root cause of pump failures was due to foreign debris such as oil sludge in the engine oil. GM stated that this debris can become trapped by the oil inlet filter screen. Under normal operation, this screen allows a small amount of oil into the brake vacuum pump. The foreign debris can, over time, clog the pump which can cause a loss of lubrication and a reduction in the amount of vacuum produced leading to a hard pedal and extended stopping distance.On December 13, 2018, GM issued a Special Coverage (N182202780) for the brake vacuum
