Loss of motive power due to engine failure post recall remedy

The Office of Defects Investigation (ODI) has received 36 Vehicle Owner Questionnaires (VOQs) alleging engine failure in vehicles subject to Recall 25V274. In each VOQ, the engine had the recall remedy completed prior to the reported failure incident. Recall 25V-274 addresses vehicles equipped with a L87 6.2L V8 gas engine that experienced engine damage or failure. Recall 25V-274’s remedy includes an engine inspection.  Based on the results of the inspection the vehicle will receive an oil change to a higher viscosity oil or an engine replacement.  The complaints received allege failures of both remedies. ODI is opening this Recall Query (RQ) to assess the adequacy of the remedy for Recall 25V-274. Loss of motive power due to engine failure

The Office of Defects Investigation (ODI) has received 1,157 reports of engine bearing failure failures in the L87 6.2L engine produced by General Motors (GM). Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019–2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. The subject engine failures were investigated as part of a Preliminary Evaluation (PE25001).  GM determined that these engine failures were due to multiple supplier manufacturing and quality issues. GM issued recall 25V-274 to address the supplier manufacturing and quality issues produced between March 1, 2021, and May 31, 2024. Based on the issuance of recall 25V-274, NHTSA has closed PE25001. ODI continues to receive a significant number of reports of engine failure in vehicles outside the scope of 25V-274.  This presents a potential safety risk that warrants further investigation.  ODI is opening this Engineering Analysis (EA) to further evaluate the scope and severity of the potential problem and to fully assess the potential s

DTN Air Bag Inflator Rupture

The Office of Defects Investigation (ODI) has become aware of eight vehicle crashes in which a rupture of a Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN) air bag inflator occurred during the deployment of the driver side air bag. All eight drivers in these crashes sustained serious or fatal injuries that appear linked to the ruptures. Six of those drivers sustained fatal injuries and two sustained severe injuries. In all eight incidents, the subject inflators had been installed as replacement equipment after the vehicle was involved in a previous crash. Information gathered during NHTSA’s initial analysis of these incidents indicates that the inflators were potentially imported illegally into the United States. In conjunction with this safety investigation, NHTSA is also working with appropriate authorities to address any illegal activities associated with the importation of these inflators. Loss of motive power due to engine failure

On January 16, 2025, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE25001 after receiving complaints and field reports concerning engine failures in MY2019-2024 GM’s full-size truck and SUV T1XX platform equipped with the L87 6.2L V8 engine. These complainants alleged engine bearing failures resulting in knocking and growling noises from the engine’s bottom end and in some cases more severe consequences such as broken connecting rods leading to engine damage, or engine failure. If the engine fails during vehicle operation, the vehicle will lose propulsion, increasing the risk of a crash. On April 24, 2025, GM issued recall 25V-274 to address the concern with L87 engines produced between March 1, 2021, and May 31, 2024.  GM cited the root cause as a supplier manufacturing and quality issues. GM’s  recall remedy will be either an oil change to an oil with a different viscosity or engine replacement depending on the results of the remedy inspection procedure. To date, ODI has received 1,157 allegations of engine bearing failure. GM submitted 55,464 reports in total, with 29,752 unique VINs. ODI continues to receive allegations of L87 engine failures

Electrical arcing and thermal events, potentially leading to fires, while utilizing Enel X Way USA, LLC's JuiceBox residential charger.

On October 3, 2024, the Office of Defects Investigations (ODI) opened PE24028 to investigate alleged failures of Enel X Way's JuiceBox Level 2 residential EV chargers. The allegations were described as thermal incidents and electrical malfunctions, which could trip the circuit breakers in Juicebox owners’ homes and disrupt electric vehicle charge cycles. On October 2, 2024, Enel X Way posted on its website that Enel X Way North America would cease operations in the United States and Canada on October 11, 2024. The closure would potentially impact the charging app and supporting software. ODI sent Enel X Way an Information Request (IR) letter on October 21, 2024. Enel X Way provided an incomplete response to the IR on December 9, 2024. Enel X Way later provided a complete response to the IR on January 24, 2025. ODI reviewed Enel X Way’s IR submissions to determine whether further action is required. Based on ODI’s analysis, there was no clear trend in the cause of the failures described in customer complaints and warranty reports. Pedestrian alert sounds

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. Air Bag Malfunction

On April 9, 2021, the Office of Defects Investigations (ODI) opened PE21-010 to investigate instances of air bag malfunction indicator light (MIL) illumination and potential for frontal air bag non-deployment in several vehicles manufactured by General Motors (GM). The vehicles included model year (MY) 2020 through 2021 CT4 and CT5 passenger cars, Sierra and Silverado trucks (1500/2500/3500 models), XT4 and MY 2021 Escalade, Suburban, Tahoe, Yukon SUVs.Technical Service Bulletin (TSB) 21-NA-005, issued in March 2021, addresses air bag MIL illumination with diagnostic trouble codes (DTCs) B0001-1B or B0012-0D. It cites rust particles in the connection terminal interface of the driver air bag inflator housing as the cause of the air bag MIL illumination. Illumination of the air bag MIL under these circumstances may result in a non-deployment of the frontal driver air bag during a frontal collision and increased risk of injury to the driver.GM stated a correlation exists between production delays due to the pandemic shutdown and premature corrosion initiation on the surface of the air bag inflator housing, which, in some cases, caused rust development during a seven (7) month period o

Electric Vehicle Battery Fires

In October 2020, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE) on 2017-2020 Model Year (MY) Chevrolet Bolt vehicles based on complaints received alleging that the vehicles caught fire under the seating area while parked and unattended. At the time, ODI had received 2 Vehicle Owner Questionnaires (VOQs) and several field reports. All complaints alleged vehicles caught fire during or after electrical charging.ODI's review found vehicle thermal events occurred during and/or at the end of the battery module charging cycle. The information learned from NHTSA and General Motors joint vehicle inspections prompted General Motors to file Recall 20V-701 on April 29, 2021. The remedy for this recall included 1) performing diagnostic procedures to identify and replace potentially defective battery cell-module assemblies and 2) installing onboard diagnostic software, designed to detect and warn owners regarding potential issues related to changes in battery module performance over time. Front Wiper Transmission Inoperative

On December 13, 2019, the Office of Defects Investigation (ODI) upgraded RQ18-002 to an Engineering Analysis (EA19-005) to further assess the MY2010-2016 Chevrolet Equinox and GMC Terrain windshield wiper failures. Due to a prior recall, 16V-582, addressing this issue in the MY2013 subject vehicles, MY2013 was not included in the analysis. To date, ODI had received 998 complaints with allegations of the front windshield wiper failing in adverse whether conditions, while the vehicle was in motion.General Motors (GM) initiated Safety Recall 16V-582 on August 3, 2016 to address windshield wiper failure in the Model Year (MY) 2013 Chevrolet Equinox and GMC Terrain. Failures were attributed to water and debris intruding into the windshield wiper assembly ball joints, leading to excessive wear and eventual detachment of the affected ball joint resulting in loss of windshield wiper function. ODI?s review focused on 2010-2012 and 2014-2016 Equinox and Terrain vehicles. Owners were reporting a high rate of failures of the windshield wiper system. Loss of Power Steering Assist

In May 2019 the Office of Defects Investigation (ODI) opened a Recall Query (RQ19-001) to assess the scope of General Motors (GM) recall 16V-054. ODI had received 50 complaints with allegations of loss of electric power steering assist while the vehicle was in motion. These complaints were for vehicles outside the scope of recall 16V-054 yet exhibited the same symptoms.ODI?s review focused on specific build months outside the recall scope that had a high electric power steering failure rate and failures that were the same or similar to those addressed by the remedy associated with recall 16V-054. ODI?s review of data supplied by GM confirmed a high failure rate for vehicles outside of the recall scope.On March 25, 2021, GM filed a defect information report recalling select 2015 Chevrolet Colorado/GMC Canyon vehicles to remedy a potential loss of power steering assist while driving. The recall (21V-213) includes vehicles that were not addressed by recall 16V-054 and also prompted this recall query.In view of the recall, ODI is closing this recall query. Despite closing this investigation, NHTSA reserves the right to take additional action if warranted by future circumstances.The ODI

Brake Vacuum Pump Failure

On November 20, 2018, the Office of Defects Investigations (ODI) opened PE18-012 to investigate instances of sudden onset of a hard brake pedal that could result in extended stopping distance due to an issue with the brake assist vacuum pump in model year (MY) 2014 through 2016 GMC Sierra 1500 and Chevrolet Silverado 1500 series pickup trucks and MY 2015 through 2016 Cadillac Escalade, Chevrolet Suburban, Chevrolet Tahoe, and GMC Yukon SUVs.If the brake assist vacuum pump fails to operate as intended, the amount of brake power assist supplied to the driver can be significantly reduced, extending the subject vehicle stopping distance. GM stated that the root cause of pump failures was due to foreign debris such as oil sludge in the engine oil. GM stated that this debris can become trapped by the oil inlet filter screen. Under normal operation, this screen allows a small amount of oil into the brake vacuum pump. The foreign debris can, over time, clog the pump which can cause a loss of lubrication and a reduction in the amount of vacuum produced leading to a hard pedal and extended stopping distance.On December 13, 2018, GM issued a Special Coverage (N182202780) for the brake vacuum

Windshield Wiper Transmission Failure

On November 2, 2018, the Office of Defects Investigation (ODI) opened a Recall Query (RQ18-002) to assess the scope of Recall 16V-582.General Motors (GM) initiated Safety Recall 16V-582 on August 3, 2016 to address windshield wiper failure in the Model Year (MY) 2013 Chevrolet Equinox and GMC Terrain. Failures were attributed to water and debris intrusion into the windshield wiper assembly ball joints, leading to excessive wear and eventual detachment of the affected ball joint with an attendant loss of windshield wiper function. The recall remedy addressed these conditions with the installation of an improved windshield wiper motor/transmission assembly and the relocation of a drain hole on the Air Inlet Panel at the base of the windshield.RQ18-002 was opened to assess windshield wiper failure rates in the MY 2010-2012 and MY 2014 - 2016 Equinox and Terrain (subject vehicles).During the course of the investigation, ODI received additional consumer complaint traffic bringing its total to 602 concerning the alleged defect in the subject vehicles.
