Recall 23V-413 Post Remedy Failures
On June 8, 2023, Fiat Chrysler Automobiles US, LLC (FCA) filed Safety Recall 23V-413 affecting certain 2022â2023 Jeep Grand Cherokee and2021â2023 Jeep Grand Cherokee L vehicles to address rear coil springs that may not have been properly installed during vehicle production. An improperly installed rear coil spring may detach while the vehicle is in motion, creating a road hazard that can increase the risk of a crash for following motorists and may result in injury to other road users. Since the filing of Recall 23V-413, the National Highway Traffic Safety Administration (NHTSA) has received twenty (20) unique consumer complaints alleging rear coil spring detachments occurring after the recall remedy was performed. These reports raise concerns regarding the effectiveness of the recall remedy and suggest the possibility of additional factors not addressed by the original recall campaign. Accordingly, the Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to assess the adequacy of FCAâs remedy procedure under Recall 23V-413 and to evaluate the root cause(s) of post-remedy coil spring detachments. Instrument Panel Cluster (IPC) Failure
The Office of Defects Investigations (ODI) is opening a Preliminary Evaluation on 2020 Model Year Jeep Gladiators and Wranglers involving a malfunctioning Instrument Panel Clusters (IPC) resulting in the loss of important vehicle metrics and other status information. ODI has received 89 Vehicle Owner Questionnaires (VOQs) alleging a complete or partial loss of the IPC display while driving.  The subject vehicles' IPC display includes the speedometer, fuel gauge, engine temperature, turn signal indicators, warning light, and other gauges that are normally illuminated while the vehicle is in operation. Loss of the fuel gauge while driving may lead to an inability to determine the fuel level and result in a loss of motive power, and the loss of other safety-related information may impede a driverâs ability to take action to prevent a safety risk.  Furthermore, an IPC that is suddenly not visible to the driver may divert the driverâs attention from the driving task, potentially causing a vehicle crash. Underhood fires
The Office of Defects Investigation (ODI) has received 9 Vehicle Owner's Questionnaire (VOQ) reports, 1 Death and Injury report, and multiple field reports alleging incidents of engine compartment fires in model year (MY) 2021-2023 Jeep Wrangler and Gladiator vehicles. The majority of the reports describe a fire occurring while the ignition in the vehicle was in an âOFFâ state with a suspected origin at the passenger front side of the engine compartment. An ignition âOFFâ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. ODI has contacted the vehicle manufacturer, Fiat Chrysler Automobiles (FCA), and learned of several thermal events originating at the power steering pump electrical connector in recent model year Jeep Wrangler and Gladiator vehicles. The location of the pump connector is located at the passenger front side of the engine compartment, coinciding with the majority of fire reports known to ODI. Engine Stall With Intermittent Restart
The Office of Defects Investigation (ODI) has received 80 consumer complaints alleging an engine stall and loss of motive power on model year (MY) 2022 RAM 1500 pickup trucks and 2022 Jeep Wagoneer SUV vehicles. The particular engines in the subject vehicles are equipped with the Fiat Chrysler Automotive Group LLC (FCA) 5.7L Hemi eTorque system. The eTorque system is a mild hybrid system that lets the vehicle partially power itself. It accomplishes this by converting the captured energy from the braking system into electricity for the battery pack, which is then used to power a variety of electrical components. ETorque engines replace the alternator with a 48-volt battery-powered belt drive and a motor generator. The motor generator provides extra torque to the crankshaft during gear changes. Simultaneously, the motor generator uses the 48-volt battery pack to increase torque. Many of the complaints state that the vehicle's engine would shut off, often while travelling at slow speed, the vehicle would shift into park and apply the emergency brake. Loss of Motive Power
The Office of Defects Investigation (ODI) has identified 68 Vehicle Owner Questionnaire (VOQ) reports for model years (MY) 2021 through 2024 Jeep Wrangler 4xe vehicles alleging a loss of motive power. Recall 22V-865 was issued by the manufacturer to address an engine shutdown condition in certain MY 2021-2023 Wrangler 4xe vehicles. The complaints noted here contain both post recall remedy failures and failures in vehicles not included within the scope of 22V-865. An analysis comparing complaints received before the issuance of recall 22V-865 and complaints outside the scope of the recall has been conducted. The results show the current complaint rate is higher in the out of scope vehicles than it was for vehicles that were later covered by the recall. ODI is opening this Recall Query (RQ) to assess the severity of the alleged defect in vehicles not included in recall 22V-865.ODI will also assess the scope and remedy effectiveness of the recall. High pressure fuel pump failure
The Office of Defects Investigation (ODI) opened PE21-021 on October 14, 2021, to investigate incidents alleging a stall/loss of motive power as a result of high-pressure fuel pump failures in certain model year (MY) 2019-2020 Ram 2500, 3500, 4500, and 5500 heavy duty trucks equipped with 6.7L Cummins turbodiesel engines. During the investigation, ODI sought to determine if the related defect allegation was limited in scope to the recalled population. After review of information request response materials from both FCA and BMW, NHTSA determined that sufficient information to identify a comprehensive recall population could not be produced by FCA and BMW.During the investigation FCA filed recalls 22V406, 22E048, 22V767, and 22E087 which include vehicles not identified in the initial subject population by ODI. ODI also received recall 21V586, involving loss of motive power due to failed CP4 fuel pumps on certain BMW manufactured vehicles. An information request letter response received from BMW indicated that failed pumps on their vehicles were caused by an interaction between pump internal components and US market diesel fuel, leading to increased slip and eventual particle-generati
Pedestrian alert sounds
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
False overheat leading to loss of motive power
On December 2, 2022, the Office of Defects Investigation (ODI) opened PE22-013 to investigate allegations of loss of motive power accompanied by high coolant temperature messages on the dashboard of Model Year 2019-2020 Jeep Compass vehicles. At the time this investigation was opened, ODI was aware of fifteen Vehicle Owner Questionnaires (VOQs) for Model Year (MY) 2019-2020 Jeep Compass vehicles alleging a loss of motive power (LOMP) while driving. The LOMP events were often accompanied by a high coolant temperature or coolant temperature too high message appearing on the dashboard of the vehicle.  In most incidents, the high coolant temperature or coolant temperature too high dashboard messages spontaneously self-corrected or were corrected after the vehicle was stopped and restarted. One complainant alleged no immediate restart, and the vehicle was towed.  A Preliminary Evaluation (PE) was opened to assess the frequency and potential safety consequences of these engine shut down events. On January 19th, 2023, ODI sent an information request letter to Fiat Chrysler US LLC (FCA) requesting pertinent information on the subject 2019-2020 Jeep Compass vehicles.  FCA provided its
Electronic Parking Brake Water Ingress
The Office of Defects Investigation (ODI) opened investigation PE22-009 after having received 80 complaints on model years 2014-2020 Jeep Cherokees alleging the Electronic Parking Brake (EPB) inadvertently activated while the vehicle was in motion.During this investigation, the subject vehicles were updated to include only Model Years (MY) 2014-2016, while MY 2017-2020 were considered peers.Based on related repair invoices, ODI believed the EPB module was damaged by water leaking into the vehicle. When the EPB inadvertently activates, it can cause a stalling condition bringing the vehicle to a stop. ODI was aware of recall 15V-393 that addresses water ingress affecting the power liftgate module on nearly the same population of vehicles subject of this investigation. The liftgate module can potentially catch fire as a result of the water damage. Both the liftgate and EPB modules are located in a similar area of the vehicle. Stall from Crankshaft\Camshaft Failure
On December 13, 2016, the National Highway Traffic Safety Administration (NHTSA) received recall 16V-907 from Fiat Chrysler Automobiles (FCA) that identified a safety defect involving crankshaft or camshaft sensor failures on certain 2016 Model Year Dodge Journey, Jeep Compass, and Jeep Patriot vehicles. The failure of the crankshaft or camshaft sensor may lead to a loss of motive power (LOMP) in the recalled vehicles. Following the receipt of recall 16V-907, the Office of Defects Investigation (ODI) received 127 vehicle ownerâs questionnaires (VOQ), in which consumers allege experiencing crankshaft or camshaft failures on similar vehicles not included in the recall. On July 22, 2022, this Recall Query was opened to further assess the scope, frequency, and safety consequences of the alleged defect described in the recall. During this investigation, ODI sent two information request (IR) letters to FCA. The first letter was sent on August 15, 2022, and the second on July 12, 2024. Responses to these IR letters, that included manufacturer claim data related to the alleged defect, were received on September 26, 2022, and September 6, 2024, respectively. Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. Seat Belt Retraction
The Office of Defects Investigation (ODI) opened this Preliminary Evaluation to investigate complaints alleging improper retraction of the front seat belts (driver or passenger) due to the plastic guide of the D-ring cracking in model year (MY) 2014 Jeep Grand Cherokee vehicles. The purpose of the investigation was to understand the scope, frequency and safety related consequence of the alleged defect. The subject condition occurs when the plastic material encapsulating the metal substructure of the front seat belt, B-pillar mounted D-ring cracks. The structural integrity of the D-ring remains intact; however, cracking of the plastic material pinches the webbing at the D-ring, thereby preventing it from properly retracting, and leaving the belt loose on the occupant. In addition to the detectability of the loose belt, the cracks in the plastic guide are also apparent by visual inspection, and many reports indicate the webbing can be manually fed back into the B-pillar/retractor to improve seat belt fit. Frame weld deficiencies
On September 16, 2019, the Office of Defects Investigation (ODI) granted Defect Petition DP18-004 and opened Preliminary Evaluation PE19-012 to investigate allegations of deficient frame welds and their potential safety related effects on 2018-2019 Jeep Wranglers. Due to a previous steering related recall that was caused by a misaligned weld on the front track bar (NHTSA Recall No. 18V-675), ODI decided to include reports of steering shimmy or wobble, loose steering, and steering stiction (loss of power steering) as part of this investigation and parent petition (DP18-004). ODI received information from the vehicle manufacturer Fiat Chrysler Automobiles (FCA) via information request letters dated March 8, 2019, and February 6, 2020. Supplemental requests for information occurred between July 2020 and July 2023. A review of the responsive manufacturer information showed that vehicles presenting an unexpected reduction in steering ability due to a welding tool misalignment were correctly addressed via recall 18V-675. Active Head Rest Inadvertent Deployment
The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. Seat belt buckle bracket failure
On July 17, 2019, NHTSA's Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE) 19-008 after receiving three Vehicle Owner Questionnaire (VOQ) reports alleging the driver's side seat belt buckle separated from the seat frame, rendering the seat belt non-functional. The VOQs involved model year (MY) 2015 and 2016 Jeep Wrangler (JK platform) that were configured for right-hand drive (RHD) use. On August 8, 2019, ODI issued an information request (IR) letter to FCA.The scope of the IR included all JK Wrangler produced for sale in the U.S. (both left- and right-hand drive), and amongst other things, requested any reports or allegations of driver's seat belt bracket failures on these vehicles. In its review of FCA's response, ODI determined that approximately 1.7 million JK Wrangler were produced from MY 2007 to 2018, about 16,000 of which were configured for RHD. ODI notes that the RHD Wrangler are typically used for rural postal delivery which often experience a more severe duty cycle. Electrical overstress
The Office of Defects Investigation (ODI) opened this investigation to determine if the failure of airbags to deploy during severe crashes, in certain vehicles, was the result of a safety related defect. During the investigation a complex failure was studied that can result in non-deployment of subject vehicle air bags and other restraint system devices in severe crash events. The subject vehicles may be equipped with an airbag control unit (ACU) for the supplemental restraint system (SRS) Electronic Control Unit (ECU) manufactured by ZF-TRW.  The ECU receives signals from crash sensors mounted in the vehicle and deploys the vehicle air bags and seat belt pretensioners in accordance with manufacturer design specifications.  The ECU in the subject vehicles contains a model DS84 application-specific integrated circuit (ASIC) which controls the communication of the crash sensor signal, firing commands (i.e., when to deploy the airbag(s) and/or pretensioners), and fault information (e.g., diagnostic trouble codes). In September 2016, FCA announced recall 16V-668 for certain model year (MY) 2010 to 2014 Chrysler, Dodge and Jeep products manufactured with the subject ZF-TRW ACU.  In
Frame Weld Deficiencies
In a letter received on October 24, 2018, a citizen petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a safety defect investigation into frame weld deficiencies on model year (MY) 2018 'JL' Jeep Wrangler vehicles. The petitioner described a broad list of frame weld deficiencies, including porous welds, excessive slag, lack of weld penetration, over penetration of welds, overweld or weld drip, and weld splash. The weld defects are allegedly located at a variety of locations on the frame assembly.On November 16, 2018, the Office of Defects Investigation (ODI) opened a Defect Petition (DP18-004) to evaluate whether to grant or deny the petition. Northwest Chrysler Jeep Dodge Sale Issue
NHTSA opened AQ17-004 to determine whether Northwest Chrysler Jeep Dodge of Houston, Texas (Northwest) has complied with the requirements of the National Traffic and Motor Vehicle Safety Act, 49 U.S.C. Chapter 301 (Safety Act) and its implementing regulations. The Safety Act requires, among other things, that a manufacturer notify its dealers of defects related to motor vehicle safety and non-compliances with Federal Motor Vehicle Safety Standards and, in turn, that a dealer not sell a new vehicle subject to a recall unless the recall remedy has been performed (49 U.S.C. 30120(i)). NHTSA issued information request letters (IRs) to Northwest and Fiat Chrysler Automobiles US LLC (FCA) in August 2017, and again in October 2017 and December 2017. NHTSA's investigation indicated that Northwest sold and delivered at least 310 recalled vehicles that did not have the recall remedy completed at the time the vehicles were delivered to the customers. On August 19, 2020, NHTSA entered into a Settlement Agreement with Northwest and its parent company, Ken Garff Automotive LLC, to resolve the Audit Query. On April 22, 2016, Fiat Chrysler Automotive US LLC, (FCA) submitted a Defect Information Report (DIR) to NHTSA regarding a defect that could result in unattended vehicle rollaway with the engine running in approximately 811,586 model year (MY) 2012 to 2014 Dodge Charger and Chrysler 300 vehicles and MY 2014 to 2015 Jeep Grand Cherokee vehicles equipped with 8-speed transmissions and a monostable gear selector (NHTSA Recall #16V-240, FCA Recall #S27). According to FCA's DIR, The existing strategies built into these vehicles to deter drivers from exiting the vehicle after failing to put the transmission into PARK have not stopped some from doing so. Powered vehicle rollaway
The Office of Defects Investigation (ODI) opened Preliminary Evaluation PE15-030 on August 20, 2015,to investigate 14 complaints alleging incidents of rollaway after intended shifts to Park in Model Year (MY) 2014 and 2015 Jeep Grand Cherokees vehicles. The MY 2014-2015 Grand Cherokee vehicles are equipped with Monostable electronic (E-shift) gearshift assemblies supplied by ZF Group (ZF). The E-shift system operates electronically and the gear requested by the driver is transmitted from the shifter via the CAN Bus to the Transmission Control Module which makes the requested shift. The Monostable gearshift does not move into a detent but springs back to a centered/neutral position after the driver selects a gear and releases the shifter. A button on the shift knob must be depressed to shift out of Park, shift out of Neutral, and to shift from Drive to Reverse or Park.The gear selected is shown on a display in the dash and illuminated letters on the shifter. If the driver's door is opened when the gearshift is not in Park, a chime sounds and a message is displayed on the instrument panel to warn the driver.In addition, the engine Start/Stop push-button control logic does not permit

On July 23, 2015, Fiat Chrysler Automobiles (FCA) launched Safety Recall 15V-461 to remedy security vulnerabilities in approximately 1.4 million model year (MY) 2013 through 2015 vehicles equipped with Uconnect head units (HU) 8.4A (RA3 radio) and 8.4AN (RA4 radio) manufactured by Harman International.On July 24, 2015, the Office of Defects Investigation (ODI) opened Recall Query, RQ 15-004, to investigate HU security vulnerabilities and remedy effectiveness in the recalled population and to determine whether similar units have been supplied for use in other FCA vehicles.In an August 11, 2015 letter, FCA submitted a second Part 573 safety recall report expanding the scope of the Uconnect RA4 model radio to include additional 7,810 MY 2015 Jeep Renegade vehicles manufactured from September 18, 2014 through June 25, 2015 (Recall 15V-508).Scope analysis indicated that Uconnect radios installed in FCA vehicles not included in recalls 15V-461 or 15V-508 (subject recalls) are not equipped with built-in cellular access or short range wireless communication features and, thus, do not contain the security vulnerabilities addressed by the subject recalls. False Positive Automated Braking
On June 1, 2015, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE15-021 to investigate 9 complaints alleging incidents of unintended braking in model year (MY) 2014 Jeep Grand Cherokee vehicles, including 2 alleging incidents of full ABS braking.ODI expanded the scope to include MY 2015 Grand Cherokee vehicles and identified a total of 176 incidents in complaints provided by FCA or submitted to ODI by consumers.ODI conducted the following work to assess the alleged defect in the subject vehicles:1) evaluated the design of the subject Forward Collision Warning (FCW) system, including the maximum braking authority during automated braking; 2) analyzed the frequency and severity of field incidents of unintended braking; 3) analyzed warranty data for evidence of system faults that may be related to the alleged defect; and 4) reviewed the performance of the subject system in testing conducted as part of NHTSA?s 2014 Automatic Emergency Braking (AEB) test program.ODI?s analysis found that the maximum brake activations that can be commanded by the subject system are limited in both braking force and duration.The system is not capable of achieving full ABS braking
Engine compartment fire
On October 15, 2015, Fiat Chrysler Automobiles, US LLC, (FCA), submitted a Defect Information Report (DIR) to NHTSA regarding an air-conditioning suction/discharge combo line routing condition that may result in engine compartment fire in approximately 75,574 model year (MY) 2015 Jeep Cherokee vehicles equipped with 2.4L engines and built from October 1, 2014 through June 18, 2015 (NHTSA Recall 15V-676, FCA Recall R57).According to FCA's DIR, a misrouted A/C combo line can be pinched and cut or thermally degraded by the exhaust manifold heat shield, potentially resulting in loss of air conditioning, smoke and/or fire from ignition of leaked A/C refrigerant oil onto a hot exhaust manifold.The recalled vehicles will be inspected for misrouted A/C combo lines and those found to be misrouted will be replaced.FCA estimates that one percent of the recalled population may be affected by the A/C combo line routing defect.ODI?s analysis of warranty data provided by FCA in response to the information request letter for PE15-003 identified a spike in claims related to the A/C combo line leakage in vehicles equipped with 2.4L engines built after October 1, 2014.FCA implemented its final proces
