Inner Tie Rod Failures

The Office of Defects Investigation (ODI) has received one Vehicle Owner Questionnaire (VOQ) and Early Warning Reporting (EWR) field report data on 2021 Nissan Rogue vehicles with allegations of inner tie rod failures resulting in a loss of steering control. The failures have been reported to occur on either driver or passenger side of the vehicle. In some cases, the vehicle required towing from the failure location for repair service. ODI is opening this investigation to further assess the scope, frequency, root cause, and potential safety-related consequences of the alleged defect leading to loss of steering control on 2021 Nissan Rogue. Side curtain air bags may deploy inadvertently

The Office of Defects Investigation (ODI) has received multiple Early Warning Reporting (EWR) field reports alleging inadvertent deployment of the side curtain air bags in Model Year (MY) 2015 Nissan Rogue Select vehicles.  The field reports allege the side curtain air bags inadvertently deploy after the door is shut and/or slammed.  Inadvertent deployment of an air bag can cause occupants to be injured or result in the loss of protection from the air bag when needed in a crash. ODI is opening this Preliminary Evaluation (PE) to assess the scope, frequency, and severity of the inadvertent deployment allegations.

Reduced Power After Engine Stall

The Office of Defects Investigation (ODI) opened this investigation on March 7, 2024, after receiving twenty (20) consumer complaints and multiple field reports describing reduced power (limp mode) after re-starting the vehicle following a stall on certain 2022-2024 model year (MY) Nissan Versa.  A Preliminary Evaluation was opened to gather information and assess whether the subject vehicles contain a safety-related defect. During this investigation, Nissan explained that certain 2022-2044 MY Nissan Versa vehicles equipped with a manual transmission may, under specific conditions, experience a Malfunction Indicator Light (MIL) along with reduced engine performance following an engine start-up after a stall.  In certain conditions, if the vehicle stalls and an engine restart occurs within five (5) seconds, the vehicle will enter a fail-safe mode with limited engine torque. The fail-safe mode helps prevent damage to the engine, a complete loss of motive power, and allows the driver to maintain control of the vehicle. ODI reviewed over 180 incidents where drivers described entering a limp mode following a vehicle re-start after having stalled the vehicle.  Review of these incident

Complete loss of motive power due to engine failure

On December 13, 2023, the Office of Defects Investigation (ODI) opened PE23-023 to investigate allegations of engine failures leading to loss of motive power in model year (MY) 2021-2023 Nissan Rogue; MY 2019-2021 Nissan Altima; and MY 2019-2021 Infiniti QX50 vehicles. At that time, ODI had received 6 Vehicle Owner Questionnaires (VOQ) and multiple early warning reports, with consumers reporting engine knocking noises accompanied by a loss of engine power while driving. The investigation identified bearing failures in certain vehicles equipped with the subject 3-cylinder 1.5L or 4-cylinder 2.0L variable compression turbo (VC-Turbo) engines. ODI also identified early warning reports describing thermal incidents that occurred immediately after the vehicle suffered a catastrophic engine failure.  The failures were the result of a manufacturing defect in specific engine bearings (main, A-, C-, and L-link) or other engine components that may cause engine damage and potentially lead to engine failure. Loss of motive power due to broken crankshaft with no ability to restart.

On November 15, 2023, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE23020 to investigate allegations of loss of motive power in model year (MY) 2016 through 2019 Nissan Titan XD trucks equipped with 5.0L Cummins diesel engines.  The complaints allege the condition was due to a failure of the engine crankshaft.  The purpose of the investigation was to understand the scope, frequency, and safety related consequence of the alleged defect. During the investigation, it was found that the subject vehicles can experience a loss of motive power due to a failure of the crankshaft main bearings which can lead to a fractured crankshaft. This resulted in a loss of motive power and an inability to restart. An analysis of all complaint sources shows many vehicles exhibit knocking, vibration, malfunction indicator lamp (MIL) warnings, and/or poor engine performance prior to a total failure.  Some consumers report driving their truck in for repairs only to be diagnosed with a failed crankshaft.  ODI has not identified any injuries or fires attributable to this failure. Pedestrian alert sounds

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

Hertz/Vehicles Rented with Open Recalls

NHTSA is opening this audit query to investigate whether The Hertz Corporation (Hertz) has complied with the requirements of the National Traffic and Motor Vehicle Safety Act (Safety Act). The Safety Act requires, among other things, that a rental company shall not rent a vehicle subject to a safety recall unless the recall remedy has been performed. 49 U.S.C. § 30120(i).NHTSA is in receipt of information that indicates Hertz rented vehicles to customers without having performed required recall repairs. Accordingly, we are opening this audit query to seek additional information concerning this issue.

Hood Latch/Bell Crank Assembly

The Office of Defects Investigation (ODI) opened this preliminary evaluation to investigate consumer complaints alleging hood latch failures occurring on certain model year (MY) 2013-2016 Nissan Pathfinder vehicles. Complaints received indicated a sudden and unexpected opening of the hood while the vehicle was in motion, blocking the driver's visibility.During this investigation Nissan collected parts from the field to analyze for root cause of the alleged failures. On June 13, 2022, Nissan submitted a Defect Information Report (DIR) to NHTSA regarding a hood bell crank assembly defect in certain MY2013-2016 Nissan Pathfinder vehicles produced at the Smyrna, TN plant. According to Nissan's DIR, an accumulation of dust and dirt contamination on and around the bell crank lever pivot joint, which actuates the secondary hood latch, can create a condition that can cause the lever to remain in the open position after it has been disengaged. In this condition, the secondary hood latch may not hold the hood closed as designed while the vehicle is in motion obstructing the driver's forward view and increasing the risk of a crash.A remedy plan for all affected vehicles is currently under dev

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. False Automatic Emergency Braking

NHTSA received a defect petition (DP19-001) from the Center for Auto Safety (CAS) on March 21, 2019 alleging that the Automatic Emergency Braking (AEB) system on Model Year (MY) 2017-2018 Nissan Rogue and Rogue Sport vehicles was engaging in cases where there was no obstruction in the path of the vehicle. The CAS petition additionally alleges that, based on Nissan's actions, the manufacturer is aware of the issue due to issuing a Technical Service Bulletin (TSB) launching two "Quality Actions," and initiating a "Customer Service Initiative" in relation to it. CAS contends that Nissan's actions do not represent an adequate long-term solution to the problem, as the action is only available during the basic coverage period of the warranty. Rear Suspension Control Arm Failure

On July 29, 2019, the Office of Defects Investigation (ODI) opened EA19002 to investigate instances of alleged rear lower control arm failures in 2013-2018 Nissan Altima and 2016-2018 Nissan Maxima vehicles. At the time the investigation was opened ODI had 91 consumer complaints alleging rear lower control arm failures in the subject vehicles.ODI was concerned about these complaints because many of them alleged an increased steering effort or loss of vehicle control.At the time these complaints were received, ODI was also aware of a proactive service campaign addressing 2013 Nissan Altima in salt-belt states for a similar concern. Since opening the investigation, ODI has confirmed, based on consumer complaints, warranty data, and testing conducted by Nissan and ODI, that owners of the subject vehicles are experiencing a corroded bushing at one of three mounting points of the control arm to the rear subframe.ODI has also confirmed that drivers become aware of the problem because of sound and abnormal vibrations. Rear Suspension Control Arm Failure

PE18-013 was opened on December 1, 2018, with 4 complaints to ODI alleging incidents of the lower control arm fracturing at a connection point to the vehicle chassis due to corrosion. ODI has received an additional 87 complaints since the PE was opened. During PE18-013, Nissan indicated model years 2013-2018 shared a common rear lower control arm design. Nissan provided complaint and warranty data regarding 48 incidents of the subject failure. The majority of the complaints are from vehicles registered in salt belt states. There have been no crashes, injuries, or fatalities confirmed as of this date. Nissan advised that it has conducted testing to assess safety risk. Nissan states that due to low incident rate, high detectability, and low risk of adverse vehicle dynamics, it does not believe the subject lower control arm failure poses an unreasonable risk to motor vehicle safety.This preliminary evaluation has been upgraded to Engineering Analysis (EA) 19-002. Pedal Operation Interference

On August 11, 2015, Nissan North America, Inc., submitted a Defect Information Report (DIR) describing a condition that may impede the driver's ability to transition quickly and smoothly from the accelerator pedal to the brake pedal in certain model year (MY) 2012 through 2015 Nissan Versa Sedans and MY 2014 through 2015 Nissan Versa Note vehicles (NHTSA Recall No. 15V-507).According to Nissan's DIR, in certain circumstances related to driver foot position and shoe type, the right edge of the driver?s shoe may catch the edge of the center console lower trim panel.Nissan indicated that this condition could cause a slight delay in the smooth transition between the accelerator pedal and the brake pedal which may increase the braking distance; therefore increasing the risk of a crash.Nissan's recall instructs dealers to trim the console panel so that the leading edge is at a greater distance from the driver?s foot.Nissan had previously issued a service campaign (Nissan Campaign ID #P5308; TSB #NTB15-033) on April 16, 2015, to provide the remedy for the same set of vehicles before deciding to change the program to a safety recall.Of the approximately 421,305 vehicles covered by recall 1
