ELECTRICAL SYSTEM,UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE — Filed 08/13/2026

1. The telematics control module failed and was replaced by the dealer under recall N262547330, along with the battery. The vehicle is available for inspection. The original module was kept by the dealer when it was replaced. 2. The vehicle went completely dead without warning — it could not be unlocked or started. It happened at home, but had it happened on the road I would have been stranded. The keyless entry also no longer works, so I cannot reliably get into the vehicle. 3. Yes. The dealer confirmed the telematics module failure and replaced it, and replaced the battery. The ongoing problems — failed software updates, "no cellular signal" message, check engine light, and keyless entry failure — have not been diagnosed yet. 4. The vehicle was inspected and repaired by [XXX] Chevrolet GMC in [XXX]. GM's customer resolution team is involved in the case but has not physically inspected the vehicle. No police or insurance inspection. 5. The navigation/GPS was glitching intermittently before the vehicle went dead in August 2026. The last successful software update was XXX — every update since then has failed. The check engine light came on at pickup on XXX. The "no cellular signal" message appears every time an update is attempted. The keyless entry stopped working recently. VEHICLE SPEED CONTROL,BACK OVER PREVENTION,FORWARD COLLISION AVOIDANCE — Filed 10/03/2026

The driver pulled into a parking space at a local pharmacy. The space directly faced the building and was separated from the building by a sidewalk that was approximately 6 feet wide. After pulling into the parking space and releasing the accelerator (the vehicle was in the single pedal driving mode, so the motor braking should have been activated) the vehicle instead experienced three problems/failures. (1) Sudden Unintended Acceleration - The vehicle suddenly lurched forward without driver input. (2) The Automatic Emergency Braking System failed to activate or to override the acceleration command, even though the vehicle was directly facing the wall of the building, which was only a few feet away. (3) The Collision Warning System failed to activate. The system should have displayed a large red box with an X through it in the Heads Up Display to warn of the pending building impact. No warning was received.

On XXX I was driving a rental car that I was placed in due to my vehicle being repaired. I was placed in a Chevy Trailblazer VIN#: [XXX] My rental with this Chevy Trailblazer began on XXX. Using the available features in the vehicle, the center console has an area that you can place your cellular phone to charge your cellular phone. Which is used this option on XXX. On XXX, I placed my cellular phone in the charging bank location while I was operating the vehicle. When I arrived at my destination I took my cup out of the cup holder and grabbed the keys, and grabbed my phone and tucked it against my arm to get out the car and the charging bank my cell phone case so hot that it burnt my forearm. SERVICE BRAKES — Filed 09/22/2026

The braking system failed to communicate with the vehicles computer system. This communication failure occurred while driving and set off all kinds of alarms and codes inside the vehicle while driving and then I had to use both feet to apply the breaks in order to get the car to stop in the emergency lane on the road as we were driving. No other warning signs or messages prior to this happening everything happened all at once. Had Onstar run diagnostics on the vehicle and 2 codes came back a U1610 Communication code of the vehicles network. The network provides a link between the systems inside the vehicle and when interrupted a code can be established. Service within 7 days. Also code U1639 same description. I had the vehicle towed to the dealer and they advised there was a module that required a further update from when I had my vehicle in on 9/2 for an oil change

POWER TRAIN — Filed 11/02/2025

The transmission failed while driving on about 11/2/2025, with myself and a child in the car. It was since fixed at a dealer and is not available from me for inspection. I lost the ability to accelerate normally. I stopped at a stop light then when I accelerated to enter the intersection it did not accelerate as predicted and I got stuck entering the intersection. There was a burning smell. Another motorist helped push the car to the side and there was a large transmission fluid spot on the ground and leaking. The dealer confirmed a sudden transmission leak occurred, and transmission components were damaged by lack of lubrication and associated heat. There was no warning. My research revealed this is a known problem with this car and other GM models with this transmission and there is an internal service advisory about bolts that loosen and sudden transmission failure. I confronted GM Customer service on this and they reimbursed less than my cost for rental and gave an extended warranty on rebuilt transmission. I had asked for a new one.

POWER TRAIN — Filed 05/17/2026

My vehicle is a 2025 Chevrolet Equinox EV equipped with a stalk-mounted electronic shift selector. On multiple occasions, when shifting from Reverse(R) to Drive(D), the transmission has failed to actually engage Drive despite the shifter indicating the D position had been selected. In most instances this resulted in the vehicle to unexpectedly move backward, requiring me to re-select the gear. This defect caused a collision. I was attempting to Dirve out of a parking space and needed to move quickly. I shifted the stalk to Drive, and — consistent with normal driving behavior — did not look at the instrument panel to visually confirm the gear had engaged before accelerating, as drivers do not typically re-verify gear selection after shifting. The vehicle had not actually engaged Drive. When I applied the accelerator, the vehicle did not move as expected, and I struck a vehicle behind me. The collision caused damage to my vehicle's rear hatch requiring full hatch replacement, estimated at approximately $4,000. There was no visible damage to the other vehicle. This is not an isolated occurrence for my vehicle — the shifter has intermittently failed to register a gear change from R to D on other occasions, typically resulting in only a few inches of unintended travel in the wrong direction rather than a collision. I reported this issue verbally to my dealership's service department, and was told they have received other reports of similar shifting problems with this vehicle, though I have not yet had a formal repair order completed for this specific issue. I am filing this complaint because a shift selector that does not reliably engage a gear is a safety defect: drivers reasonably rely on the shifter position without independently verifying engagement via the instrument cluster before applying the accelerator, and this can result in unintended vehicle movement in the wrong direction, including collisions, which it did in my car

SERVICE BRAKES,ENGINE,FORWARD COLLISION AVOIDANCE — Filed 09/03/2026

On 3 occasions, the last happening today (XXX) traveling north on XXX toward [XXX] at approximately 14:05, the vehicle has stopped abruptly and hard, and will then release. Today, it took it longer to release. When it occurred this time, it braked just as hard but longer, so I had to cross two lanes so the cars behind me would not hit me. The cars swerved around me and the car eventually let go and I was able to accelerate from that dangerous position on the side of the highway. The first time it happened, someone was pretty close behind me. The car stopped hard, real hard, but let go quickly enough to accelerate before a collision. The second time, there was no one in front or behind us, and we were safe. It hadn’t happened in a while, today. We’ve downloaded 2 or 3 software update, and I was hoping it fixed the issue. It did not. Today the car stumbled and the rpm’s jumped. I feel like the car momentarily braked while I was accelerating. SERVICE BRAKES, HYDRAULIC — Filed 09/02/2025

The contact owns a 2025 Chevrolet Equinox. The contact stated that while driving approximately 15 MPH or below and coming to a stop, the brake pedal was depressed, but the vehicle failed to respond as intended, and the brake pedal was pumped several times until the vehicle responded. The contact stated that the failure mostly occurred while driving on wet, gravel, and snowy roadways. The contact stated that on one occasion the vehicle drove through an intersection. The vehicle was taken to a dealer twice; however, the failure was not reproduced on the first visit. During the second visit, the contact was informed that the failure might be related to a cruise control recall, and a software update was completed; however, the failure persisted. The failure mileage was approximately 2,000.

POWER TRAIN — Filed 03/11/2026

I've noticed several times that my '25 Equinox LT AWD doesn't shift gears correctly, like it gets stuck on a certain gear. When it gets stuck, I slowly take my foot off the accelerator, it often helps, but this needs to be fixed. I had one scary incident when I was getting on the freeway and accelerating of course and the car automatically shifted to Neutral. Thank goodness there was no cars behind me, I had to pull over and switch it to drive. This is one of the times I took it to the dealership. I've taken it to the dealership several times for this issue, but they say since they can't duplicate that issue there is nothing they can do. I'm in an Equinox group and there's quite a bit of people that are having this same issue.

VISIBILITY/WIPER,FORWARD COLLISION AVOIDANCE — Filed 04/16/2026

The Chevrolet Equinox EV has a large blind spot because of the size of the front window pillars and the adjacent side view mirror. To quote from an Internet search, "This creates a significant blind spot when preparing to make left at intersections, making it difficult to spot pedestrians cyclists, or oncoming traffic." My vehicle was involved in an accident during a left turn in which I believe the blind spot was a significant factor. The Chevrolet Equinox EV is supposed to be equipped with "enhanced automatic emergency braking" which, if so, did not function to prevent an accident.

SERVICE BRAKES,FORWARD COLLISION AVOIDANCE — Filed 03/15/2026

While pulling into a parking spot of a business and approximately 6 feet from the curb, the automatic emergency brake engaged forcing my whole body forward, slamming my head on the visor/roof of the car very forcefully/painfully. I was not in traffic, I was getting ready to park. My front seat passenger slammed against her seatbelt and I had a child in the back seat who slammed forward as well. No dash indicators or seat rumbling occurred, it was just an immediate jolt/hard stop that I was not braced for. At risk of concussion or TBI.

UNKNOWN OR OTHER — Filed 01/13/2026

After hitting a deer, the body shop fixed the damage to the front part of the vehicle. They realized the battery kept being drained even after purchasing a new one. It’s now at a dealership and they are staying it needs a new ONSTAR module, not sure when it will come in either. I’m paying for a rent car that I can’t afford to pay for anymore. I wanted them to just do away with the ONSTAR but they said it’s not possible.

SERVICE BRAKES,FORWARD COLLISION AVOIDANCE — Filed 12/10/2025

SEE NHTSA Safety Recall Report 25V-12- It appears it should apply to both 2025 FRONT WHEEL DRIVE Chevy Equinox EVs with ADAPTIVE CRUISE CONTROL (and without Super Cruise) in addition to 2025 ALL WHEEL DRIVE Equinox EVs with ACC (also without Super Cruise). This is very dangerous. Here's what happened: My 8 month old 2025 Chevrolet Equinox EV FRONT WHEEL DRIVE w/ACC(no Super Cruise) has had 2 instances where a very large "thunk" sound and vibration emanating from the rear of the car while stopped at an intersection, was immediately followed with sudden brake failure and sudden forward acceleration. The ACC was on in both instances. The first time this happened was alarming but since the was nothing in front of me there was no damage and the problem seemed to resolve itself thereafter. The second time was while waiting at a red light about 10 feet behind another vehicle. My car shot forward and into the other car, which was then pushed into the car ahead of it. Both my car and the car ahead of me had very expensive damages, but fortunately, no injuries. What I experienced sounds exactly like the recall problem described in NHTSA Safety Recall Report 25V-12 which has thus far only been applied to the AWD version of the 2025 Equinox EV, not the FWD version that I have. I believe this needs rapid consideration due to the danger of of sudden acceleration resulting in serious property damage, injury, or death. It appears to me that other instances of this problem have been described in various online forums and YouTubes.

SERVICE BRAKES,FORWARD COLLISION AVOIDANCE — Filed 11/07/2025

I was driving my 2025 Chevrolet Equinox EV on the highway with Adaptive Cruise Control (ACC) engaged. The vehicle was also set to "One-Pedal Driving" (High regenerative braking). The vehicle in front of me slowed down abruptly. My vehicle's ACC began to slow the car, but it was not decelerating quickly enough to avoid a collision. I immediately intervened by pressing the brake pedal as hard as I could, pushing it completely to the floor. Critical Safety Failure: The vehicle's physical brakes failed to engage. Despite my foot being pressed hard to the floor, the vehicle did not slow down. It felt as though only the regenerative braking was (slowly) working and that my manual brake application had zero effect. The vehicle continued to glide forward with no significant braking power, and I was unable to stop the car in time, resulting in a collision with the rear of the vehicle ahead. Summary of Defect: The primary safety failure was the complete lack of response from the manual brake pedal. My attempt to override the vehicle's automated systems in an emergency was unsuccessful, as the brake pedal was unresponsive. Reporting History: I have already reported this incident to my auto insurance, my local Chevrolet dealership, and the Chevrolet Customer Assistance Center. A representative from Chevrolet recommended that I file this report with the NHTSA to help identify and track similar safety-critical incidents.

FORWARD COLLISION AVOIDANCE — Filed 10/09/2025

on 10/9/2025 I pulled out of my driveway and less than 300 feet from my house I struck a deer. Weather was approx 50 degree and clear. There was no dew on my windows as I park my car in the garage. There was approximately $6200 worth of damage done to my vehicle and what concerned me the most was, the car never engaged the emergency stop. I mentioned this to the insurance company and they really had nothing to say. I have had bicyclists get too close to me on the road and my vehicle engaged the emergency stop. After repairs were done on the vehicle, they had a third party come in to do a calibration of all the sensors. He could not perform the calibration due to #PIT6274: Error Message Referring to Vision Camera system with DTC B1C48 - (Nov 4, 2024). I just dropped my vehicle off at the dealer and mentioned this to them and they are claiming there is nothing they can do about it. I started looking around online and have seen several other people complain about this and I knew it affected my Adaptive Cruise Control as I have gotten this error a couple times before and I just thought my window was dirty. What I did not know is this also affected my emergency stop as it does not tell me that in the warning. If this is a known issue, this is a serious safety concern and to have the dealer tell me there is nothing they can do is not acceptable. If the camera was functioning properly that morning, there is a good chance that I would not have gotten into an accident.

FUEL/PROPULSION SYSTEM,BACK OVER PREVENTION,FORWARD COLLISION AVOIDANCE — Filed 09/15/2025

Vehicle was being parked in garage when it accelerated in reverse, on it's own, very quickly. Fortunately I was able to avoid colliding with any thing behind me by braking in time. I then shifted into drive when vehicle accelerated forward, on it's own again, very quickly. I was not able to brake before vehicle crashed into concrete garage wall, severely damaging front end. Vehicle is still under warranty, and I intend to have dealership haul it in for repairs. I have found other drivers complain on the Internet of experiencing the exact same issue as I described above. This model, Chevrolet, Equinox, 2025 is not safe to drive!
