SERVICE BRAKES,FORWARD COLLISION AVOIDANCE — Filed 06/16/2026
What component or system failed or malfunctioned, and is it available for inspection upon request? The automatic braking turned on while I was accelerating. My car is available for inspection. How was your safety or the safety of others put at risk? I could have been rear ended because my car automatically braked at an unpredictable time in rush hour traffic with someone very close behind me. Has the problem been reproduced or confirmed by a dealer or independent service center? It has happened to me maybe 4 times before but the dealer hasn't been able to reproduce it. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? It first happened soon after I bought my car new.
STRUCTURE,FUEL/PROPULSION SYSTEM — Filed 08/21/2025
[XXX] fuel door assembly has come loose from body. It looks like the clips that hold it in place have become worn or loose by the weight of the gas nozzle or by the fact that the gas door will not close correctly and car wash machine has made it looser and it appears to not shut all the way, and the body colored panel has fallen off as a result and when you pump gas, the assembly moves may be possibly causing it to leak gasoline if the seal or gasket becomes dislodged because of the loose clips that hold assembly to the body panel. ELECTRICAL SYSTEM — Filed 04/13/2026
The vehicle intermittently fails to start when pressing the push-button start system. The start/stop button becomes unresponsive despite normal conditions (key present, brake engaged, no warning indicators preventing ignition). This has now occurred twice within two years of ownership, indicating a recurring issue rather than an isolated incident. On the most recent occurrence, the vehicle was completely unable to start and required towing. The tow truck operator reported that he had towed four vehicles of the same make/model for the exact same issue within one week, suggesting a broader pattern affecting multiple vehicles. Safety Concern: This presents a significant safety risk, including: •Inability to start the vehicle in unsafe or emergency situations •Risk of being stranded in dangerous environments (e.g., roadside, extreme weather, unsafe areas) •Potential failure of the ignition system, which raises concerns about reliability of vehicle operation overall A malfunction in the start/stop system directly impacts the vehicle’s basic functionality and creates an unpredictable and potentially hazardous situation for drivers. Additional Notes: Given the repeated occurrence and reports of similar failures in other identical vehicles, this issue may represent a systemic defect that warrants investigation.
VISIBILITY/WIPER — Filed 04/06/2026
I was driving down the Highway and the sun roof just exploded send shards of glass everywhere and nothing had hit it or anything. It just exploded. When I got to the dealer the person ahead of me had the exact same thing and we compared pictures. It was scary, dangerous and really loud when it happened.
ELECTRICAL SYSTEM,VEHICLE SPEED CONTROL,SERVICE BRAKES — Filed 02/18/2026
I was sold this 2024 Mazda cx30 from Ken ganley kia located in mentor Ohio (44060) the vehicle started showing major electrical issues. when they occur I have no control of the functions of anything in the cabin (infotainment, heat/air, braking system,) just to name a few. My wife and I were assured we were purchasing a good safe and reliable used vehicle. I feel this vehicle is extremely unsafe and unpredictable and ken ganley kia of mentor refuse to take this unsafe vehicle back or even pay for the repairs. I am utterly disappointed such a big name in the auto industry could turn there back on a customer not only that let a young family pull off in a unpredictable electrical nightmare of a vehicle and refuse to stand by it.
LANE DEPARTURE,FORWARD COLLISION AVOIDANCE — Filed 11/12/2025
Cruise control and lane keeping system engaged. Vehicle will occasionally aggressively apply brakes, when nothing visible is in front of the vehicle. This has nearly cause our car to be rear ended on a number of occasions. I suspect the system mistakenly interprets a building or vehicle shadow as a physical object. This response has also occurred when vehicle in front (cruise control distance at slower speed) turns right. Again, nearly causing vehicle behind to be rear ended.
POWER TRAIN — Filed 01/01/2026
I had both the left front and right front CV axles fail. My car only has 11.5k miles on. I first heard the clunking noise at 11k miles, went right to the dealer and had them watch me recreate the noise and they determined the axles needed to be replaced. This is incredible dangerous as the CV axle could have been completely disconnected from the wheel assembly leaving my car useless and uncontrollable.
SERVICE BRAKES, HYDRAULIC,AIR BAGS — Filed 11/30/2025
The contact rented a 2024 Mazda CX-30. The contact stated that while driving at highway speeds, an "i-ACTIVSENSE Partially Disabled Front Radar Malfunction" warning message appeared on the instrument panel. The contact attempted to exit the highway; however, the vehicle failed to stop upon depression of the brake pedal. The contact continued to pump the brakes and even engaged the emergency brakes, but the vehicle failed to stop. Due to the failure, the contact crashed into the vehicle in front of her as the "Vehicle System Malfunction, Have the Vehicle Inspected" warning message appeared on the instrument panel. The air bags did not deploy upon impact. The contact later sought medical attention for pain in her back; the other driver sustained injuries; however, the injuries are unknown as she did not seek medical attention from the paramedics that arrived at the scene. A police report was filed. The vehicle was towed from the scene to an independent mechanic, where it remained in their possession. Neither the dealer nor the manufacturer had yet been notified of the failure. The vehicle was not repaired. The failure mileage was 38.377.
UNKNOWN OR OTHER — Filed 10/15/2025
Component/System: Automatic transmission (requires full replacement). The vehicle is currently located at Safford Mazda of Fredericksburg, 10653 Patriot Hwy, Fredericksburg, VA, 22408, and is available for inspection upon request. Safety risk: The transmission developed a loud whining and grinding noise while driving, indicating imminent risk of sudden loss of motive power or seizure. This creates a severe crash hazard, especially on highways or during acceleration/merging. Sudden loss of propulsion without warning can leave the driver unable to accelerate, maintain speed, or move out of harm’s way. The problem has been reproduced and confirmed by Safford Mazda (authorized Mazda dealer). On October 16, 2025, they diagnosed the vehicle as requiring a complete transmission replacement (Repair Order/Multi-Point Inspection Video is available). The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives, but remains at the dealership awaiting parts. Starting approximately in late August, a transmission whine started zero warning lights or alerts ever appearing. Noise rapidly worsened. [XXX]: expert friend drove it once, instantly recognized imminent failure, and ordered me to stop driving immediately. At ~20,000 miles, the dealer confirmed the drive unit must be fully replaced, still with no dashboard warning ever triggered. Had I kept driving while trusting the silent system, this critical component could have seized at any moment, causing sudden total loss of propulsion at highway speed or in traffic with my young child on board. The complete absence of any alert during a proven, progressive failure is an urgent, unacceptable safety defect that demands immediate investigation and recall before a family is injured or killed. As of [XXX] , the vehicle has been out of service for 33+ calendar days. The dealer informed me I am #60 in Mazda’s national transmission replacement backlog. AIR BAGS,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE — Filed 10/09/2025
While driving normally at highway speed on I-880 South, the braking system failed to decelerate the vehicle. When the brake pedal was pressed firmly, there was no normal slowing—only a sliding sensation, as if the wheels locked and the car moved forward on ice. This led to a collision with a stopped vehicle ahead. The following day, the Mazda App displayed a red alert message: “Brake system malfunction. Braking effectiveness may be decreased.” The vehicle’s airbags deployed very forcefully, causing bruising to both driver and passenger, even though the impact was not severe. Three days before the incident, the vehicle had a scheduled maintenance and recall service at a Mazda dealer, including a brake inspection and air-bag recall repair. The vehicle was declared a total loss by the insurance company and is currently stored at Copart Hayward, CA. The problem has not been inspected by Mazda yet, but a formal safety review is requested. The issue posed serious safety risks due to total loss of braking control and unexpected air-bag deployment.
