AIR BAGS — Filed 09/20/2026

I was driving home from church late Sunday night. A really rough storm hit and it knocked down a huge tree in the road covering the entire road. I did not see the tree until I got up on it and I went straight through it. I was running around 40 mph and total front end impact took the whole bumper off but my airbags did not deploy.

TIRES — Filed 10/31/2024

A whole new set of 4 tires were purchased and installed on the vehicle on 10/10/2024. While driving at highway speed 3 weeks later the rear driver side tire, in quick succession, went flat and almost instantaneously disintegrated causing the car to spin out and crash off the road. The car airbags deployed- saving the driver from injury or worse. Since the incident occurred out of home state and far from home and the car was totaled - it was sold as junked and not available for inspection. The accident could have involved other vehicles if the road was more crowded. Since only a single vehicle was involved - Maryland police would not write or provide a report. We believe that the tire failed on its own and wasn’t caused by an external object.

My brother was driving his car and it was involved in a hard right front impact yesterday after a young girl pulled out in front of him. None of the airbags went off, severe front end damage on the right side to the fender, wheel, axle, radiator, hood, oil pan, and other front end components.The right front wheel is completely bent and the right front tire blew out. Front right headlight completely missing. Incident happened going approximately 30-40 MPH. Thankfully he’s okay, but the car is most likely totaled. The steering wheel airbag did NOT deploy and the right curtain airbag did NOT deploy. My brother could have gotten killed. Thankfully he’s only complaining about leg pain and chest pain, but the steering wheel airbag would’ve mitigated his injuries. A few years ago, he got sideswiped very hard on the left side and the vehicle needed 2 new doors and a rear bumper. The left side curtain airbags did not deploy in this scenario either. I’m very disappointed in Toyota that they built a death trap such as the 2018 Corolla.

ELECTRICAL SYSTEM — Filed 07/17/2022

The contact stated that after parking the vehicle in the driveway, approximately 12 hours later while the contact was sleeping, a sound woke the contact and it was discovered that the vehicle caught on fire. The fire department was called to the scene and extinguished the flames. During the incident, the vehicle was destroyed and the fire also caused damage to the garage door, the side of the home, and the concrete driveway. The contact mentioned that the battery was replaced by AAA 3 weeks after the incident. No injuries were reported. A fire report was taken at the scene. The vehicle was later towed away. The cause of the fire was not determined. The manufacturer and local dealer were not notified of the fire. The failure mileage was 8,000.

The contact owned a 2018 Toyota Corolla. The contact stated while driving into a parking space, the vehicle accelerated without any warning causing the vehicle to drive over the curb and crash into a 7 eleven store building. The air bags did not deploy, and no one was injured. The contact was unsure if there was a police report filed. The vehicle was towed, and it was deemed for total loss. The dealer was contacted and stated they were unsure of the cost of the repairs because the vehicle had an anti-lock brake feature. The manufacturer was contacted, and a case was filed. The failure mileage was 16,500.

FUEL SYSTEM, GASOLINE — Filed 04/11/2022

The contact owns a 2018 Toyota Corolla. The contact stated that while driving at various speeds, the brake warning and the master warning lights illuminated intermittently. The vehicle was taken to an independent mechanic where a diagnostic reader was used, however, the mechanic was unable to locate a failure code. The vehicle was not repaired. Additionally, the contact stated that after restarting and refueling, the vehicle was stalling and although she depressed the accelerator pedal, the vehicle delayed accelerating. The contact referenced failure to NHTSA Campaign Number: 20V012000 (Fuel System, Gasoline) however, VIN was not included in the recall. The contact then stated that after the vehicle was refueled and she came to a left turn to make a U-turn, the vehicle was stalling and then jerked forward independently, causing her to crash into the back of an attached trailer. The airbags failed to deploy. There was an additional passenger in the vehicle, however, the contact stated that there were no injuries and that a police report was not filed. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 19,000.

The contact owned a 2018 Toyota Corolla. The contact stated that while driving at 60 MPH in inclement weather, the brakes failed to stop the vehicle and she lost control of the vehicle. The vehicle crashed into the median and was spinning before another vehicle crashed into the contact's front end. The air bags were not deployed upon impact. The contact suffered soreness in her arms, legs, and back; however, no medical attention was received. A police report was not filed. The vehicle was towed to an auto collision center and deemed totaled by the insurance company. The manufacturer was made aware of the failure and a case was opened. The failure mileage was 100,555.

The contact stated no one was injured or sought medical attention. A police report was filed. The contact was able to drive to her residence but continue to experience the failure. The contact took the vehicle to a collision center and had the bumper replaced. The mechanic informed that he had also experienced the failure and referred the contact to a local dealer. The contact took the vehicle to a local dealer but was unable to duplicate the failure. The contact traded the vehicle for another vehicle. The manufacturer had not been informed of the failure. The failure mileage was approximately 78,000.

MY CAR HAD EARLY SIGNS OF PROBLEMS WITHIN ONLY THE SECOND YEAR. IT HAD STOPPED TWO TIMES. IF THIS CAR STOPS BEFORE THE 18,000 OR LEMON LAW RETURN IT. I THEN WAS NOTIFIED THAT IT HAD 3 RECALLS. MY CAR WAS ACTUALLY STILL DRIVEABLE. I ALSO WANT TO DISCLOSE THAT I DEALL WITH ENVIROMENTAL ALLERGIES. I HAD THE VEHICLE FIXED SEVERAL TIMES WITH NO PROBLEM. HOWEVER, AFTER THE RECALLS WERE FIXED WITH THE FUEL PUMP AND AIR BAG I STARTED TO BECOME EXTREMLY ILL . I EVER PURCHASED A VOC MONITOR AND WAS ABLET TO DETECT GAS AT DANGEROUS LEVELS. MY DEALER TOYOTA BOWIE AND THE MANUFACTURER DID LITTLE TO NOTHING TO ACCOMODATE ME. I WILL NEVER BUY ANOTHER TOYOTA AGAIN, OR A CAR PERIOD UNLESS I CAN PAY FOR IT IN CASH UPFRONT. THE VEHICLES VOC TURNED DANGEROUS AFTER TURNING ON THE CAR FOR 10MINUTES STRAIGHT.

THE VEHICLE THEN STALLED AND THE CONTACT WAS ABLE TO COAST TO THE SIDE OF THE ROAD TO WAIT FOR THE DEALER TECHNICIAN TO PICK UP THE VEHICLE. THE CONTACT STATED THAT THE TECHNICIAN HAD SPRAYED A SUBSTANCE ON THE ENGINE IN ORDER TO START THE VEHICLE AND TAKEN IT TO THE DEALER BUT WHEN ATTEMPTING TO START THE VEHICLE, THE VEHICLE TO IGNITE INTO FLAMES. THE CONTACT STATED THAT THE VEHICLE WAS DESTROYED DUE TO NEGLIGENT FROM THE DEALER. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND OPENED A CASE. THE FAILURE MILEAGE WAS APPROXIMATELY 24,000.

ON 9-9-20 I WAS HEADING HOME FROM THE BANK, DID NOT FEEL WELL, LOST CONTROL OF THE CAR AND RAN OVER A ROAD DIVIDER ISLAND CURB AT THE INTERSECTION OF LAKE MERCED BLVD. AND SKYLINE BLVD., S.F. AT AROUND 5 PM. THE FRONT AIR BAGS DID NOT DEPLOY WHEN I THINK THEY SHOULD HAVE IS WHAT I AM REPORTING HERE. MY SON AND I WERE IN A HEAD ON COLLISION. THE OTHER CAR WAS AT FAULT. SPEED FOR BOTH VEHICLES WAS AROUND 35 MPH. MY 11 YEAR OLD SON WAS IN THE REAR PASSENGER SEAT BUCKLED. I WAS DRIVING. I HAVE A FEW BRUISES BUT AM OK. MY SON IS NOT. HE HAS MULTIPLE FRACTURES TO HIS HEAD (RIGHT SIDE), RIGHT EYE SOCKET, SPINE (UPPER NECK AND BACK). AS YOU CAN SEE FROM THE PICTURES HE SHOULD NOT HAVE THESE INJURIES. THE INSURANCE ADJUSTER SAID I NEED TO CONTACT AN INVESTIGATOR TO SEE WHAT WAS WRONG WITH THE AIRBAG TO CAUSE THESE INJURIES.. WE ARE LOOKING AT WEEKS OF HOSPITAL AND INPATIENT REHAB FOR HIM.

THE MANUFACTURER WAS CONTACTED AND WERE CONDUCTING AN INVESTIGATION. POLICE REPORT NUMBER: 19049540 WAS FILED. THE VEHICLE WAS DESTROYED AND TOWED. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE DEALER WAS NOT NOTIFIED. THE APPROXIMATE FAILURE MILEAGE WAS 30,000.

ON SUNDAY, OCTOBER 20, 2019, APPROXIMATELY 3:10 PM I GOT INTO MY 2018 TOYOTA COROLLA, THE CAR WAS STATIONARY IN PARK, AND WAS NOT RUNNING ON A FLAT SURFACE. I REALIZED I FORGOT MY WALLET SO I GOT OUT OF THE CAR AND LEFT THE DRIVER'S DOOR OPEN. I RETRIEVED MY WALLET WITHIN 1 MINUTE AND WHEN I CAME OUT MY CAR WAS ROLLING SLOWLY ON THE FLAT DRIVEWAY WHERE I PARK EVERY DAY. I TRIED TO GRAB THE DOOR HANDLE TO SEE IF I COULD JUMP IN THE CAR TO STOP IT BUT IT PICKED UP SPEED AND WENT DOWN A SLIGHT INCLINE ACROSS THE YARD. THE DOOR HIT A TREE AND THEN THE CAR LANDED IN BUSHES. MY NEIGHBORS CAME RUNNING OVER BECAUSE THEY SAW THE ENTIRE INCIDENT. I WAS ABLE TO GET INTO THE CAR ON THE PASSENGER SIDE AND I STARTED THE CAR'S ENGINE SINCE IT WAS NEVER TURNED ON AND DROVE IT OUT OF THE BUSHES BACK ONTO THE FLAT DRIVEWAY. THE CAR ROLLED WHILE THE GEAR WAS IN PARK AND WAS NOT RUNNING. EVERYONE IS NOT SURE WHY THAT WOULD HAPPEN. I'M ASKING TO SEE IF THERE IS A PROBLEM WITH THE TRANSMISSION OR THE GEAR SLIPPING OUT OF GEAR. I CALLED MY INSURANCE THEY FILED A CLAIM BUT STATED IT WAS VERY STRANGE FOR THE CAR TO HAVE ROLLED AND TOLD ME TO FIND OUT IF THERE WAS A RECALL, ALSO THE ADJUSTER SAID IT WAS VERY STRANGE AS WELL FOR THE CAR TO ROLL IN PARK AND NOT RUNNING. THEY SUGGESTED IT MAY BE SOMETHING WITH THE COMPUTER SYSTEM.

THERE WERE NO INJURIES. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO A COLLISION LOT WHERE IT WAS DEEMED A TOTAL LOSS. THE MANUFACTURER WAS CONTACTED AND PROVIDED CASE NUMBER: 1908261236. THE DEALER WAS NOT CONTACTED. THE FAILURE MILEAGE WAS APPROXIMATELY 15,438.

4/20/19, THE DRIVER INVOLVED WITH THIS VEHICLE LOST CONTROL AND HIT A CURB AT 25 MILE PER HOUR, WELL WITHIN THE SPEED LIMIT AT 45 MPH. AFTER HITTING THE CURB, THE VEHICLE SWERVED INTO A TREE. THE FRONT END OF VEHICLE HIT THE TREE AND THE ENTIRE FRONT END OF VEHICLE WAS SEVERELY DAMAGED BEYOND REPAIR. THIS IS CONSIDERED A "TOTAL LOST." THE VEHICLE WAS EQUIPPED WITH FRONT AND SIDE AIRBAGS, BUT SURPRISINGLY NONE WAS DEPLOYED.
