Inadvertent Deployment of Air Bags
The Office of Defects Investigation (ODI) received a Petition dated May 30, 2026. The Petition is requesting an investigation into inadvertent air bag deployment in model year (MY) 2011-2017 Honda Odyssey vehicles. The Petition contends that air bags can deploy absent a sufficiently severe event to trigger a deployment, such as an impact, rollover event, or G-force threshold breach. The Petition further alleges that the Diagnostic Trouble Codes (DTCs) and Supplemental Restraint System (SRS) conflict and inaccurate data is being recorded for the air bag system. The Petitioner cites 10 complaints received by ODI alleging the same failure. ODI will evaluate these complaints to determine if they are related to the failure cited in the Petition andconduct a technical review under 49 CFR Part 552. This Defect Petition is being opened to evaluate whether to grant or deny the Petition. The Petition can be viewed at NHTSA.gov under NHTSA Number 11744118.
Inadvertent Deployment of Side Air Bags
On October 28, 2025, the Office of Defects Investigations (ODI) opened PE25018 to investigate complaints of inadvertent deployment of the side curtain and/or side thorax air bags. These complaints concern model year (MY) 2018-2022 Honda Odyssey vehicles manufactured by American Honda Motor Co. (Honda). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. In the recall filing, the company stated that â[a]s of April 2, 2026, Honda has had 130 warranty claims, 25 reports of an injury, and no reports of death related to this issue from January 24, 2017 â April 2, 2026.â Honda states that the Supplemental Restraint System (SRS) Electronic Control Unit (ECU) contains incorrect deployment parameters for the side thorax and side curtain air bags, which may cause inadvertent deployment when the vehicle encounters strong road impacts, such as driving over potholes, speed bumps, or road debris. Honda stated that the SRS control logic for the second and third rows of seating contained an insufficient deployment threshold margin, allowing signal inputs to be misinterpreted as side impacts and causing inadvertent deployment of the side th
Inaccurate Rear Passenger Seat Belt Warning Status
The Office of Defects Investigation (ODI) has received 6 complaints, and multiple Early Warning Reporting (EWR) field reports alleging inaccurate rear seat belt warning status. These allegations concern both false positive and false negative statuses in model year (MY) 2023-2024 Honda Pilot vehicles manufactured by American Honda Motor Co. (Honda). The complaints state that the rear passenger seat belt warning system is displaying inaccurate information to the driver for both second and third row seating positions. This can lead to the driver dismissing warnings regarding rear seat belt status. The inaccurate information can also cause the driver to believe rear occupants are buckled when they are not. This reliance on false information presents concerns for the safety of children and child seats located in the rear rows, especially in the third row where the buckled status is more difficult for the driver to visually confirm. ODI is opening this Preliminary Evaluation (PE) to determine the scope and severity of the potential problem and to fully assess the potential safety-related issues. Engine failure
The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline. The subject engine failures were analyzed as part of a Recall Query (RQ24013). It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751. Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation. ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.
No Restart After Auto Start/Stop Engages
On June 3, 2022, the Oï¬ce of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several ï¬eld reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traï¬c light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (âPGM-FIâ) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally
Rear-View Camera Failure
On June 26, 2024, the Oï¬ce of Defects Investigation (ODI) opened a Recall Query (RQ24011) to assess the long-term suitability of the remedy for recall 22V-867, which was ï¬led on November 23, 2022 by American Honda Motor Co. (Honda). Recall 22V-867 addressed concerns of a loss of the Rear-View Camera (RVC) function in model year (MY) 2017-2019 Honda Ridgeline vehicles. Recall 22V-867 states that the RVC wire harness was manufactured with a protective corrugated tubing which was insufficient to protect against bending wear and zip ties which were insufficiently tightened and would not keep the protective tubing in place. The consequence is that the RVC wire harness may fatigue and break after repeated opening and closing of the tailgate. This ultimately results in the failure of the RVC image to display due to a lack of connectivity. Hondaâs remedy identified in the recall filing was a replacement harness with longer protective corrugated tubing and sufficiently tightened zip ties. Inadvertent Automatic Emergency Braking
On March 7, 2024, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE24008) to assess reports of inadvertent activation of the Automatic Emergency Braking (AEB) system in model year 2019-2022 Honda Insight and 2019-2022 Honda Passport vehicles. The complainants allege activation of the AEB system with no apparent obstruction in the vehicle's path, resulting in rapid vehicle deceleration. Honda indicated that it is aware of a total of 412 reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and stated that some customers possibly had an inadequate understanding of the AEB system and its limitations. However, many consumer complaints received by ODI allege that Honda dealerships were unable to reproduce the condition or state that Honda dealerships informed the consumer that this is considered normal AEB operation. To date, ODI has received a total of 106 consumer complaints of inadvertent activation of AEB in the subject vehicles.In total, ODI reviewed 475 reports involving vehicles with unique VINs that may relate to the alleged defect. Engine failure
On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016â2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation. NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. Front Seat Belt Assemblies may be Missing the Rivet that Secures the Quick Connector and Wire Plate
On April 4, 2024, the Office of Defects Investigations (ODI) opened RQ24006 to investigate American Honda Motor Co.âs (Hondaâs) proposed remedy for recall 23V-782 as well as scope of the vehicles involved. Honda issued recall 23V-782 on November 21, 2023, which concerns certain model year (MY) 2023-2024 Honda Accord, Honda Accord Hybrid, and Honda HR-V vehicles. Recall 23V-782 states that the driver and front passenger seat belt pretensioner assemblies may have been manufactured without a rivet that secures the pretensioner quick connector and wire plate. Recall documentation states that seat belt pretensioner assemblies that are missing the rivet will not properly restrain the occupant in the event of a collision, increasing the risk of injury.Subject defective seat belt assemblies are installed as original equipment for both the driver and front passenger positions for the HR-V, but only for the driver position for the Accord and Accord Hybrid. The recall remedy for 23V-782 instructs the technician to inspect both front seat belt pretensioner assemblies and, if necessary, replace them. Rear-View Camera Failure
On February 13, 2024, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24004 to assess alleged failures of the rear-view camera (RVC) in 2020-2023 Model Year (MY) Honda Ridgeline vehicles. At this time, ODI has received a total of 50 complaints with allegations of the RVC image failing to display when the vehicle was in reverse. The RVC wire harness is routed near the hinge in the tailgate, in a location where frequent opening and closing of the tailgate causes bending stress on the harness. Honda determined the wire harness was manufactured with a material which is permeable to water and salt, which when it freezes and thaws, combined with tailgate usage, ultimately leads to breakage of the wiring in the tailgate harness. This results in a failure to display the RVC image. On May 9, 2024, Honda filed a Part 573 Safety Recall Report recalling all 2020-2024 MY Honda Ridgeline vehicles. The dealer will replace the RVC tailgate harness with a wire harness that is manufactured by a new supplier and has improved material properties to withstand wear from bending. Momentary Increased Steering Effort
On November 29, 2023, the Oï¬ce of Defects Investigation (ODI) opened this Engineering Analysis (EA23003) to assess allegations of momentary increase in steering eï¬ort in model years (MY) 2022-2023 Honda Civic, 2023MY Honda CR-V, and 2023MY Acura Integra vehicles (subject vehicles). The complaints received by ODI alleged that the subject vehicles experienced a momentary increase in required steering eï¬ort (described as âsticky steeringâ) occurring mostly at highway speeds after driving for an extended amount of time; these complaints could not be duplicated during dealer service techniciansâ quick test drives. The complaints had been received over the previous two years with most occurring with low vehicle mileage. The steering gear of these vehicles is aided by an electrical power assist unit. This power assist unit inputs force on the steering rack via a worm gear and worm wheel assembly. On March 17, 2023, The Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE23-005) to access the alleged defect of momentary increase in steering effort in model years (MY) 2022 and 2023 Honda Civic vehicles. The complaints report that the momentary increase in steering effort (described as "sticky steering") occurs mostly at highway speeds after driving for a certain amount of time. The reports have been received over the past 2 years with most occurring with low vehicle mileage. The steering gear contains a unit that includes a worm gear and a worm wheel. Honda stated this condition of momentary increase in steering effort occurs due to two factors within this unit. During manufacturing, the worm wheel goes through annealing and component conditioning processes. These processes caused internal stress and strain within the worm wheel. This strain was slowly released over the first few months of the vehicle life. Over time, the released strain caused the deformation of the teeth on the worm wheel, causing the worm gear to catch on the worm wheel. Pedestrian alert sounds
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
Rear Differential Seizure
On November 29, 2022, the Office of Defects Investigation (ODI) opened PE 22-011 after reviewing field reports alleging rear differential seizure in 2018-2022MY Honda CR-V and HR-V vehicles equipped with all wheel drive. To date, ODI has not received any consumer complaints. Some of the field reports received alleged the vehicleâs wheels locked up with no warning while the vehicle was in motion. These vehicles were towed to a dealership. Examination of these vehicles showed the driveshaft had broken and the differential unit was completely damaged. The field reports cited a lack of oil due to damaged oil seals as the cause. Honda provided data including consumer complaints, field reports, and warranty claims they received relating to the loss of oil retention and resulting differential damage. Hondaâs data cited 1201 incidents with unique vehicle identification numbers (VINs). The rear differential oil seals prevent the oil from leaking out and protect internal components from contaminants. A loss of oil lubrication results in failure of the vehicle differential. Honda stated that the oil sealâs lip contact with the driveshaft may become unstable due to intermittent chang

Rear-View Camera Failure
On February 7, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22-001) to assess alleged failures of the rear-view camera (RVC) in 2017-2018 Model Year Honda Ridgeline vehicles. At this time, ODI has received a total of 107 complaints with allegations of the RVC image failing to display when the vehicle was in reverse.The RVC wire harness is manufactured with a protective corrugated tubing and harness guide to help mitigate stress on the RVC wire harness when the tailgate was opened and closed. Honda determined the length of corrugated tubing inserted into the harness guide was insufficient to provide optimal protection for the wire harness. This lack of protection for RVC wire harness led to severe wire harness damage that ultimately led to the failure of the RVC image to display due to a lack of connectivity.On November 23, 2022, Honda filed a defect information report recalling all 2017-2019MY Honda Ridgeline vehicles. The dealer will replace the RVC tailgate harness with an improved wire harness that has longer corrugated tubing to protect the wire harness.In view of the recall action being taken by Honda, 22V-867, ODI is closing this Preliminar
Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. Loss of Direction Control
On May 6, 2021, the Office of Defects Investigation (ODI) opened Engineering Analysis (EA21-001) to assess a sudden loss of steering control, in Model Year (MY) 2013-2015 Honda Accord vehicles. Consumers alleged while driving the steering wheel would pull to the left or the right independently. Steering pull may pose a risk of loss of directional control. Honda stated that they believed the inadvertent steering input was caused by a loose magnet located within the electronic power steering (EPS) system. Due to poor formulation of the adhesive, this magnet can become loose and move up to 2 degrees around the steering column. When the magnet moves, the torque sensor generates a signal that results in unwanted steering in either direction. Honda stated the steering force required to maintain directional control with a loose magnet is a maximum of 3-pound feet. The maximum force of the unwanted steering input can be overcome by the driver and is similar to driving on grooved pavement.The Vehicle Research Test Center (VRTC) sent over 4,000 questionnaires to 2013-2015 Honda Accord vehicle owners. The Office of Defects Investigation (ODI) opened this investigation to determine if the failure of airbags to deploy during severe crashes, in certain vehicles, was the result of a safety related defect. During the investigation a complex failure was studied that can result in non-deployment of subject vehicle air bags and other restraint system devices in severe crash events. The subject vehicles may be equipped with an airbag control unit (ACU) for the supplemental restraint system (SRS) Electronic Control Unit (ECU) manufactured by ZF-TRW.  The ECU receives signals from crash sensors mounted in the vehicle and deploys the vehicle air bags and seat belt pretensioners in accordance with manufacturer design specifications.  The ECU in the subject vehicles contains a model DS84 application-specific integrated circuit (ASIC) which controls the communication of the crash sensor signal, firing commands (i.e., when to deploy the airbag(s) and/or pretensioners), and fault information (e.g., diagnostic trouble codes). In September 2016, FCA announced recall 16V-668 for certain model year (MY) 2010 to 2014 Chrysler, Dodge and Jeep products manufactured with the subject ZF-TRW ACU.  In
